Go to your local vet and get a worm pill they will kill any and all worms they cost about 10-20 dollars but they really work well !!

A chipmunk will eat all sorts of berries and nuts and if you have a bird feeder in the back yard you will most likely is them feeding Thar ,thy can easily be tamed to eat from your hands but don;t ever try to pick one up because thy will bite hard!!
