You can not make people take responsibility for their mistakes. If you feel they do not realize that they made one you can gently point out their error. You can say something like, "I don't know if you realize it or not, but when you ____, it was hurtful." If they don't then apologize you may want to limit your exposure to that person.
90% of people think they are right, all the time on everything and every subject. We who know very little encounter them here and in life. We nor you can't make them do anything. Period.
It seems that the only way you'll learn from a mistake, is that it has to hurt really, really bad. That certainly sucks for the rest of us.

Not an easy task .. And it's also not an easy task to convince some people what they did WAS a mistake. Sometimes people are very hurtful .. Purposely. That type of behaviour speaks volumns about a person's character.
You can't you can only change your own, and sometimes that may have an affect on others. If people don't think that the world will fall down around them, if they just eased up a little and were honest about their mistakes then maybe we'd live in a happier place. I could write a whole book on the mistakes I've made and continue to make, it's how you deal with them that counts. When the ego feels it's going to take a hit, it defends itself, but in the long run it doesn't really help with our integrity or our acceptance that we are all fallible.
